Planning tool
The Planning Tool has been designed to help you examine what a school’s current practices are and to provide some ideas for how a school’s transition program could be further developed. This tool is just one way for schools to record their planning. Some schools may have other methods or tools that they choose to use. The Planning Tool focuses on the key elements discussed throughout Transition Matters and aims to assist schools to consider their practices with regard to each of the important stakeholders for transition: children, parents and carers, early childhood staff and the wider community.
The Planning Tool consists of four parts:
Part 1: Identifying strengths and gaps
Part 1 aims to assist schools to look at their current transition practices and identify their strengths and any areas that could be improved.
Part 2: Focussing and prioritising
Part 2 aims to assist schools to look at their transition practices in greater detail with regards to one or more of the stakeholders.
Part 3: Action planning
Part 3 aims to assist schools to formalise their plan of action by establishing aims and setting
specific goals, brainstorm strategies, allocate resources and roles of team members, and establish
a plan to evaluate progress.
Part 4: Evaluation and reflection
Part 4 aims to assist schools to celebrate their successes, think about some of the challenges in
implementing their Action Plan, and determine where they would like to go next.
